Provider Application Form

At Ease USA (AEU) is looking for licensed trauma providers and/or organizations who practice evidence-based therapy. We are looking for providers who have experience with the military and veteran communities, and their family members. If you are interested, please complete the application to start the process to become part of the Preferred Provider Network (PPN).
